District Nursing Service
The District Nursing Service provides nursing care to people in their home. The range of services includes:
- Hygiene assessment and assistance
- Medication supervision
- Wound care management
- Breast care and support
- Monitoring and support of people with chronic illness
- Nursing care following hospital discharge
- Assessment and referral to other service providers
The District Nursing Service is able to provide nursing in the community through funding from:
- Department of Health and Human Services (DHHS) for those consumers who are under 65 who have chronic illness or disability.
- Commonwealth Home Support Packages (CHSP) for those consumers who are over the age of 65 years who have a chronic illness or disability.
The District Nursing Service also provides nursing in the community for consumers eligible for:
- Department of Veterans Affairs (DVA)
- Transport Accident Commission (TAC)
- Workcover
- Post acute care (PAC)
- Transition Care Program (TCP)
