From 16–22 March 2026, National Advance Care Planning Week invites all Australians to reflect on what matters most under the theme “Your story, your choice.”
Advance Care Planning (ACP) is an important process that helps you plan for future healthcare in case you are unable to communicate your wishes. It gives you the opportunity to consider your values, beliefs, and preferences, and ensures they are known and respected.
Why Advance Care Planning Matters:
Many people choose to undertake ACP to ease the burden on their loved ones.
As Loretta, our Advance Care Planning Coordinator, explains:
“People want to remove the burden of decision-making from family members and have peace of mind that what matters most to them will be respected.”
Loretta also encourages people not to shy away from the conversation:
“We have a 100% mortality rate, so Advance Care Planning is relevant to all of us—and talking about dying won’t make you die.”
What is Involved?
Advance Care Planning may include:
-Reflecting on your values and healthcare preferences
-Appointing a Medical Treatment Decision Maker (MTDM)
-Completing an Advance Care Directive (ACD)
In Victoria, your MTDM is the person legally appointed to make medical decisions on your behalf if you are unable to do so. This person must be over 18, willing and able to act in your best interests, and capable of making decisions in stressful situations.
An Advance Care Directive allows you to formally document your wishes and guide your MTDM and healthcare team.
