Acute Care

KDHS Acute Services provides high-quality, person-centred care close to home in a safe, supportive, and welcoming environment.

The Acute Ward includes a combination of private and shared rooms, all with ensuite facilities, designed to promote patient comfort, privacy, and recovery.

Our multidisciplinary team provides care for patients with a wide range of healthcare needs, including:

· General medical care

· Post-operative care

· Palliative and end-of-life care

· Transitional Care Program services

Patients receiving care are supported by a skilled team of nurses, doctors, and allied health professionals who work together to develop individualised care plans that meet each patient's needs and goals.

Post Acute Care

KDHS provides care services to people who need support to recover following their stay in a public hospital. The care is provided in the person’s home or at another suitable place, for example, a family member’s home.

Palliative Care

The Acute Ward includes a dedicated room for patients receiving end-of-life care.

This space provides a calm, home like setting with an adjoining lounge to provide comfort and privacy during the final stages of life and enable family members and loved ones to remained close to the patient including overnight stays

Multidisciplinary Support

Patients are supported by an experienced multidisciplinary team, including:

· Nursing staff

· Physiotherapists

· Allied Health Assistants

· Social Workers

· Dietitians

· Speech Pathologists

· Diabetes Educators

· Pharmacists

· Catering Services

· Environmental Services Staff

· Administration Staff

Together, these professionals work to support patients' medical, physical, emotional, and wellbeing needs throughout their stay.
