What is the Seniors Lifestyle Victoria program?

‍The SLV program supports older people living in the community by offering a range of services to:

‍ ‍· Improve your ability to perform everyday activities

‍ ‍· Help you to take steps to improve your health and wellbeing

‍ ‍· Empower you to make everyday choices with confidence and independence.

‍The Program supports participants for up to 6 months depending on your needs, and there are no participant fees for the period of the SLV Program.

‍ ‍

What does the program involve?

‍The program focuses on:

‍ ‍· Understanding what matters to you

‍ ‍· Focusing on what you can do, and enhancing your skills

‍ ‍· Supporting your health and wellbeing goals

‍ ‍

We work with you to:

‍ ‍· Develop and coordinate an individualised action plan based on your strengths and goals

‍ ‍· Navigate the healthcare system and access care

‍ ‍· Provide targeted supports and services that might include exercise, nutrition, medication management and opportunities for social connection

‍ ‍· Support sustained behaviour changes through ongoing assessment, monitoring and review

‍ ‍

Eligibility

‍ ‍· Do you live at home in Kyabram, Girgarre, Stanhope, Tongala or the surrounding southern Campaspe area?

‍ ‍· Are you 65 years old, OR identify as Aboriginal and Torres Strait Islander and 50 years or older?

‍ ‍· Would you benefit from access to regular check-ins, personalised help and support to maintain or improve your health, wellbeing and independence?

If you answer YES to the above questions, you can ask a health professional to refer you to the program or contact us directly (e.g. self-refer). click here to access our self-referral form.
