Our Service

The KDHS District Nursing Service (DNS) visits areas of Kyabram, Girgarre, Tongala, Merrigum and Wyuna (20 -25km radius of KDHS) to provide a professional and individualised service and aims to improve the health status and maximise the independence of clients and their carers.

Our services include:

  • My Aged Care – myagedcare.gov.au

  • Assessment and regular review of health needs

  • Monitoring and support of people with ongoing or recurring illnesses

  • Hygiene/Personal care assessment and assistance

  • Medication supervision

  • Wound care management

  • Nursing care following discharge from hospital

  • Continence care

  • Palliative care and support alongside Echuca Community Palliative Care Service and Goulburn Valley Hospice, Shepparton

  • Referral/s to other service providers

Nursing care can be provided within your home, but may also be provided at another suitable location if appropriate, for example:

  • At your workplace

  • The Urgent Care Centre at KDHS

  • The Specialist Consulting Rooms at KDHS

  • Tongala Health

  • Stanhope Health


Funding

The DNS can provide nursing care in the community through funding from:

  • Commonwealth Home Support Program (CHSP) for those aged 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) who have a chronic illness or disability

  • Home and Community Care Program (HACC) for those aged under 65 years (or under 50 years for Aboriginal or Torres Strait Islander people) who have a chronic illness or disabilit

Fees are set by the above funding bodies and can vary depending on individual circumstances and entitlements.  Please feel free to speak to us about this if there are any concerns related to fees.

The DNS also provides services for those consumers who are eligible for:

  • Department of Veterans Affairs (DVA)

  • Transport Accident Commission (TAC)

  • Work cover (WC)

  • Post-acute care (PAC)

  • Transition Care Program (TCP)

  • National Disability Insurance Scheme (NDIS)

  • Home Care Packages (HCP)

  • Hospital in the Home (HITH)

  • Private


Referrals

You, your GP, hospital or another health care provider can make a referral to the District Nursing Service. Please call us on (03) 5857 0240 for further information. If there is no answer, please leave a message and someone will return your call as soon as possible.
