What is the Seniors Lifestyle Victoria program?

The SLV program supports older people living in the community by offering a range of services to:

· Improve your ability to perform everyday activities

· Help you to take steps to improve your health and wellbeing

· Empower you to make everyday choices with confidence and independence.

The Program supports participants for up to 6 months depending on your needs, and there are no participant fees for the period of the SLV Program.

What does the program involve?

The program focuses on:

· Understanding what matters to you

· Focusing on what you can do, and enhancing your skills

· Supporting your health and wellbeing goals

We work with you to:

· Develop and coordinate an individualised action plan based on your strengths and goals

· Navigate the healthcare system and access care

· Provide targeted supports and services that might include exercise, nutrition, medication management and opportunities for social connection

· Support sustained behaviour changes through ongoing assessment, monitoring and review

Eligibility

· Do you live at home in Kyabram, Girgarre, Stanhope, Tongala or the surrounding southern Campaspe area?

· Are you 65 years old, OR identify as Aboriginal and Torres Strait Islander and 50 years or older?

· Would you benefit from access to regular check-ins, personalised help and support to maintain or improve your health, wellbeing and independence?
